Preparing for Your Appointment
- One Issue Per Visit: Each appointment is for one problem or body part. Initial visits are consultations only; procedures are not performed unless noted
- Insurance: Please confirm your plan’s network providers and coverage before scheduling.
- Emergency: If you have an emergency, call 911 instead of booking online.
Bring the following to your appointment:
- Completed paperwork
- Referral from your Primary Care Physician (if required by insurance)
- Recent X-rays, MRI films, and related test results
- A list of current medications
- Insurance card and payment method
- Notes on your symptoms and any questions for your physician
Canceling or Rescheduling Your Appointment
If you need to cancel or reschedule, please let us know as soon as possible so we can offer your spot to another patient. A fee may apply if you cancel less than 24 hours before your MRI or Physical Therapy appointment.
