Contact Us & Book an Appointment

  • Book an Appointment: Call us during business hours or schedule an appointment online.
  • Cancellations: If you need to cancel, please call at least one business day in advance so we can offer your time to patients on our waiting list.

Walk-ins: When urgent situations arise, we’ll do our best to accommodate you without an appointment. Availability may vary.

To help us serve you efficiently:

  • Complete all new patient paperwork prior to your visit.
  • Know your clinic address and location and plan for traffic or travel delays.

Please bring the following to your appointment:

  • Driver’s license or valid photo ID
  • Current insurance card(s)
    • Include co-pay, co-insurance, and deductible details, if applicable
  • Insurance referral, if required
    • Please verify our office has received your referral before your appointment; bringing a copy is recommended
  • Medical records for your current injury/condition
    • Prior X-rays, MRIs, CTs, lab results, and any relevant reports
  • List of all medications you take
    • Include name, dosage, frequency, purpose, and allergies
    • Don’t forget herbal supplements, vitamins, and over-the-counter medications
  • Emergency contact information (name, relationship, phone)
  • Something to write on for your personal notes.

If another healthcare provider has evaluated or treated you for the current issue, contact their office at least one week before your appointment to request records be sent to us and confirm we’ve received them.

Bringing a simple personal summary can make your visit more efficient. Consider including:

  • Symptom timeline: When symptoms began, how they’ve changed, and what makes them better/worse
  • Impact on daily activities/work/sports
  • Previous treatments (e.g., medications, therapy, injections, surgery)
  • Goals & questions: What you want to achieve, and what you want to ask your care team

Your personal notes are helpful but do not replace official forms you’ll complete at check-in.

  • Check in a few minutes early and complete any remaining paperwork.
  • Share your medical history, current medications, previous treatments, and referral information (if applicable).
  • Describe your symptoms clearly (onset, location, severity, function limits).
  • Physical exam: Your orthopedic specialist will examine the affected area.
  • Diagnostic testing: You may need X-rays, MRIs, or CT scans based on your evaluation.
  • Diagnosis & plan: Your doctor will explain your condition and discuss a personalized treatment plan, answering all your questions.

We know your time is valuable, and we strive to stay on schedule. However, surgeries can run long, and emergencies can occur occasionally, causing unavoidable delays.

  • If we anticipate a delay, we’ll make sincere efforts to notify you.
  • Thank you for your patience and understanding; we’ll do everything possible to keep you comfortable and informed.

Consider bringing a trusted adult to help listen, take notes, or drive if you’re uncomfortable.

If you need language assistance, please arrange an adult translator in advance or contact us prior to your appointment. (We do not recommend relying on a child to translate.)
